Summary

This form must be submitted for every project that uses an external IRB. This form is used for the UIUC HRPP to review study information that UIUC is required to review and approve.  This application is not reviewed by the IRB, although OPRS staff members conduct the HRPP review on behalf of the institution. 

Helpful Tips

No research procedures may begin until you have received an IRB approval letter from the external IRB AND an HRPP Activation Letter which will you receive related to this application form once you have provided the external IRB's approval letter and this application has been appropriately completed and reviewed.
